Kosciusko Home Care and Hospice is a not-for-profit, independent corporation, Medicare and Medicaid certified, Indiana licensed Home Care and Hospice agency.
We are located in beautiful Kosciusko County, home of over 90 lakes, rural farming communities and the “Orthopedic Capital of the World.” Our offices are in the K21 Health Services Pavilion in Warsaw.
Our agency was founded in 1976 by eight local nurses who saw a need for home care services in our community. In 1982, we added hospice services to serve the needs of the terminally ill. Our mission is to provide quality health care promoting the highest level of independence and well being to any who utilize our services. Our community’s support has been evident throughout our history. Currently, we receive financial support from K21 Health Foundation, The United Way, County Commissioners, and private donations. We look forward to continuing to provide Hometown Care for Hometown People.
