Palliative Home Care

Palliative Home Care is a new local option for Kosciusko County provided by compassionate staff who live locally.

When to refer someone for palliative Home Care:
-Serious/Advanced Chronic Illness: Cancer, Heart Disease, COPD, Dementia, Renal Disease, Liver Disease, ALS,
Parkinson’s, CHF, CKD.
-Rapid/Significant Functional Decline in the last 90 days or Nutritional Decline in the last 6 months.
-Multiple Hospitalizations/ER visits for the same condition in the last 6 months.
-Other considerations: oxygen dependent, primarily bedbound, frequent infections, dependent on 3 or more
ADLs, unmanaged symptoms, prognosis less than one year.

Goals of Care: Symptom Management, Education, Promote Quality of Life, Facilitate Advanced Care Planning,
Coordination of Care, Avoid Hospitalization.

General Eligibility Criteria: Skilled Need, Homebound, Serious Illness/Advanced Chronic Illness.

Coordination of Care: Our Palliative Home Care team collaborates with ordering physician to co-manage disease

Services Offered: Skilled Nurse, Physical Therapy, Occupational Therapy, Medical Social Work, Spiritual Counselor,
Home Health Aide.

Payors: Medicare, Medicaid, Commercial Insurance, Self Pay.

Home Care Services

Home health care offers an extensive range of health care services that can be given in your home for an illness or injury. Home health care is less expensive, more convenient, and is as effective as care you get in a hospital or skilled nursing facility.

Examples of skilled home health services include:

  • Wound care for pressure sores or a surgical wound
  • Rehabilitation Therapies which include – Physical, Occupational, & Speech
  • Patient and caregiver education
  • Intravenous or nutrition therapy
  • Injections
  • Monitoring serious illness and unstable health status
  • Medical Care for premature babies with special health concerns

The goal of home health care is to treat an illness or injury. Home health care professionals help you get better, regain your independence, maintain your dignity, and become as self-sufficient as possible.

We were the only Home Care Agency in Northern Indiana to receive this Award based on Patient Care.

Kosciusko Home Care receives 5-Star Patient Care Rating in the Fall of 2015.
